Female Hair Transplantation: Candidate Selection, Techniques & Results
A dedicated clinical study on female hair restoration, addressing Ludwig pattern thinning, traction alopecia, hairline lowering, and completely unshaven DHI.
Key points
- Female hair restoration is routinely executed completely unshaven using precision DHI implanters.
- Highly effective for Ludwig-type midline widening, traction alopecia, and cosmetic hairline lowering.
- Preoperative blood diagnostics evaluating ferritin, thyroid panels, and androgen hormones are mandatory.
1. Female Pattern Hair Loss (Ludwig Scale) & Surgical Eligibility
Female pattern hair loss (FPHL) follows a distinct pathophysiology compared to male androgenetic alopecia. While men exhibit bitemporal recession and vertex balding, women typically preserve their frontal marginal rim while developing diffuse centroparietal thinning, termed the "Christmas tree" pattern (Ludwig stages 1-3). Before surgical consideration, reversible systemic triggers must be systematically ruled out: ferritin depletion (iron deficiency), Hashimoto’s thyroiditis, polycystic ovarian syndrome (PCOS), and peri-menopausal estrogen dips. Women presenting with robust occipital donor stability, widened midline parts, traction alopecia from tight hairstyles, or naturally high foreheads represent prime surgical candidates.
2. Unshaven DHI Technique & Cosmetic Forehead Lowering
The foremost reluctance among prospective female patients is the fear of head shaving. In our Istanbul surgical facility, female procedures are executed completely unshaven via DHI Choi implanters. Recipient thinning areas remain entirely untouched by clippers. In the occipital donor zone, a micro-window is clipped beneath overlying locks, completely concealed when hair is worn down. Using needle implanters, follicles are threaded between existing terminal hairs with zero transection. Furthermore, for women with congenital high foreheads, advancing the frontal hairline by 1.5 to 2.5 cm restores harmonious facial proportions and feminine facial contouring.
3. Expected Outcomes & Clinical Success Rates in Women
With rigorous patient selection and experienced execution, female hair transplantation achieves success rates of 90% to 95%. In hairline lowering and traction alopecia cases, aesthetic transformations are dramatic, creating soft, natural feminine curvatures. In diffuse Ludwig pattern cases, synergizing DHI grafting with autologous PRP and mesotherapy dense-packs the midline, eliminating scalp show-through. Growth biology parallels male timelines: initial sprouts appear around month 3, substantial density manifests by month 6, and full styleable maturity arrives between months 12 and 15.
