First 15 Days After Hair Transplant: Recovery & Care Guide
A step-by-step clinical recovery manual for the first 15 days following hair restoration, covering vascular graft anchoring, delicate washing, and scab shedding.
Key points
- The first 48 hours represent the highest vulnerability phase where any friction must be avoided.
- Clinical hair washing commences on day 3 using specialized pH-balanced foam and lotion.
- By day 10-12, scabs are completely cleared via warm softening soaks, leaving secured follicles.
1. The Crucial First 72 Hours: Graft Survival & Sleeping Position
The 72-hour window post-transplantation is biologically delicate because newly grafted follicles lack structural vascular continuity, surviving solely via passive plasma imbibition. Minor friction, scratching, or inadvertent head bumps can dislodge shallow units. Patients must rest supine at a 45-degree inclination using a firm cervical neck travel pillow. This elevation safeguards the recipient zone from surface contact and drains intraoperative tumescent fluid posteriorly, preventing periorbital edema.
2. Days 3 Through 10: Specialized Medical Washing Protocol
On day 3, the foundational post-transplant wash is initiated in our clinical facility under nursing oversight. The goal is softening blood crusts and eliminating microbial colonizers. Panthenol foam is applied to the recipient zone and allowed to sit for 30-40 minutes. It is rinsed with lukewarm, zero-pressure water poured from a pitcher. Specialized medical shampoo is then foamed in hands and tapped vertically onto recipient grafts—no lateral rubbing or fingernails. The occipital donor site can be gently massaged. Drying is strictly performed by dabbing gently with sterile paper towels.
3. Days 10 Through 15: Scab Clearance & Permanent Graft Integration
Between postoperative days 10 and 12, the scab shedding wash marks a pivotal clinical milestone. The softened crusts, soaked under generous lotion for 45-50 minutes, are massaged away in gentle circular motions using the pulp of fingertips. Patients need not worry if hair shafts detach alongside scabs; dermal papilla stem cells are permanently integrated into the vascularized dermis. By day 15, graft dislodgement is biologically impossible, allowing a return to normal gentle washing and routine daily activities.
