Postoperative Breast Surgery Care and Compression Bra Guidelines

Author: Op. Dr. İsmail Büyükçayır · Published:

Wearing a medical compression bra continuously for at least 6 weeks post-breast augmentation, mastopexy, or reduction stabilizes implants and supports tissue redraping.

Key points

  • Medical Compression Bra: Must be worn 24/7 for the first 4–6 weeks to alleviate tension on suture lines and prevent lateral or cranial implant displacement.
  • Superior Stabilizer Band: Indicated in submuscular augmentations to encourage dynamic descent and anatomic projection into the lower breast pole.
  • Underwire Bra Prohibition: Underwire and push-up bras are strictly banned for 3 to 6 months to prevent pressure necrosis and chronic inframammary scar hypertrophy.

Biomechanical Functions of Medical Compression Bras

Postoperative mammary parenchymal tissue is hyperemic, edematous, and susceptible to gravitational shearing forces. Medical compression bras feature seamless breathable fabrics, front closures, and broad scapular straps that exert uniform pressure across the chest. This controlled compression facilitates lymphatic clearance, reduces seroma/hematoma formation, and stabilizes the implant within its dissected pocket during early capsular maturation. Over-the-counter bras cause localized focal pressure points, risking wound dehiscence.

Scar Management, Implant Massage, and Physical Restrictions

For the first 3 weeks, avoid elevating the arms above shoulder height, heavy lifting (>5 kg), and driving, as pectoralis major contractions exert strong shear forces upon the healing submuscular pocket. Once incisions are fully epithelialized (weeks 2–3), medical-grade silicone gel or sheeting should be applied to prevent hypertrophic scarring. If prescribed by the surgeon, gentle displacement massage may be introduced to foster a supple periprosthetic pocket.
