Pre-Hair Transplant Guidelines: Complete Surgical Preparation Guide

Author: Op. Dr. İsmail Büyükçayır · Published:

Essential pre-operative instructions for hair transplantation: managing anticoagulants, smoking cessation, alcohol, nutritional rules, and day-of-surgery protocols.

Key points

  • Aspirin, NSAIDs, and blood-thinning supplements must be discontinued 7 to 10 days pre-operatively under medical consent.
  • Smoking severely impairs microvascular oxygenation and must be paused at least 1 week prior to grafting.
  • Patients should not shave their hair beforehand and must wear loose-collared button-down or zippered clothing.

1. Pharmacological Screening: Discontinuing Anticoagulants

The microsurgical success of hair restoration relies on maintaining a dry, minimally bleeding surgical field during micro-punch harvesting, incision, and graft placement. Aspirin, ibuprofen, NSAIDs, Vitamin E, Omega-3 fish oil, ginkgo biloba, and concentrated green tea extracts inhibit platelet aggregation and extend bleeding times. These medications and dietary supplements must be strictly discontinued 7 to 10 days pre-operatively in coordination with the prescribing physician. Conversely, essential maintenance therapies for hypertension or diabetes are routinely taken on the surgical morning with a small sip of water after specialist clearance.

2. Abstinence Protocols: Nicotine, Alcohol & Caffeine

Nicotine and tobacco smoke induce potent peripheral vasoconstriction, drastically compromising microvascular perfusion and tissue oxygen delivery across the scalp. This microvascular deficit severely imperils graft revascularization and final yield. Patients must pause tobacco consumption at least 7 days pre-operatively. Alcohol exerts vasodilatory effects that heighten intraoperative hemorrhage; hence, it must be halted 48 to 72 hours beforehand. Furthermore, caffeine and stimulant energy drinks must be avoided on the morning of surgery to prevent transient blood pressure spikes.

3. Day-of-Surgery Protocol: Attire & Morning Nutrition

On the morning of the procedure, a light, protein-rich breakfast is recommended, avoiding greasy or heavy foods. Because hair transplantation is conducted under local anesthesia, prolonged fasting is unnecessary. The choice of clothing is critical: patients must strictly avoid tight-collared T-shirts, turtlenecks, or sweatshirts that risk rubbing against delicate freshly implanted grafts during disrobing. Loose-fitting button-down shirts or zip-up hoodies are ideal. Patients must never shave their own heads beforehand; native hair density and angles must be evaluated at full length by the surgeon before clinical trimming.

4. Clinical Screening & Health Ministry Safety Protocols

Prior to surgery in Istanbul, certified clinical laboratories perform complete blood counts (CBC), coagulation profiles (PT, aPTT, INR), and infectious disease serology (Hepatitis B, Hepatitis C, HIV). During the one-on-one consultation with the board-certified plastic surgeon, the artistic hairline, graft distribution plan, and donor conservation parameters are finalized. Rigorous pre-operative vetting ensures maximum patient safety, smooth intraoperative execution, and pristine cosmetic results.
