Tummy Tuck (Abdominoplasty): Candidacy, Muscle Repair & Recovery

Author: Op. Dr. İsmail Büyükçayır · Published:

A comprehensive medical guide on tummy tuck surgery: rectus abdominis diastasis plication, pannus excision, mini vs. full abdominoplasty, and recovery protocols.

Key points

  • Abdominoplasty eliminates persistent cutaneous pannus and repairs separated core musculature (rectus diastasis) untreatable by exercise.
  • Plication of the bilateral rectus abdominis muscles creates an internal biological corset, dramatically narrowing the waistline.
  • The surgical incision is placed low along the suprapubic line, engineered to remain entirely hidden beneath swimwear.

1. What is Abdominoplasty? Diagnostic Indications & Ideal Candidates

Abdominoplasty (tummy tuck) represents a major body contouring procedure that excises redundant, inelastic abdominal skin and resistant subcutaneous adiposity while surgically tightening the underlying musculoaponeurotic abdominal wall. It is emphatically not a bariatric weight-loss procedure, but a structural reconstructive surgery for individuals near their target body mass index (BMI). Prime candidates encompass: 1) Postpartum women suffering severe abdominal dermatochalasis and dermal striae (stretch marks). 2) Post-bariatric massive weight loss patients presenting with a pendulous abdominal apron (panniculus). 3) Individuals with rectus diastasis causing an uncorrectable protruding belly despite dedicated physical training.

2. Rectus Abdominis Plication & Waistline Sculpting

During gestation, mechanical intra-abdominal forces separate the bilateral rectus abdominis muscles laterally from the linea alba—a condition term rectus diastasis. Once aponeurotic integrity is breached, viscera project anteriorly, precluding a flat abdomen regardless of low body fat. The pivotal step of abdominoplasty is musculoaponeurotic plication: placing robust permanent braided sutures to approximate the medial borders of the rectus muscles along the midline. This reconstructs an internal biological corset, flattening the abdominal contour and narrowing the waist circumference by 5 to 10 centimeters.

3. Full vs. Mini Abdominoplasty: Comparative Paradigms

Based on the anatomical distribution of tissue laxity, two surgical techniques are utilized: 1) Full Abdominoplasty: Indicated when significant dermatochalasis and muscular diastasis span both supra-umbilical and infra-umbilical regions. The umbilicus is circumscribed and transposed into a youthful anatomic position, with the entire abdominal skin flap elevated to the costal margins. 2) Mini-Abdominoplasty: Confined strictly to patients with infra-umbilical skin laxity; the belly button is not moved and the scar remains significantly shorter. In modern protocols, both variants are routinely combined with Vaser Liposuction of the flanks (Lipoabdominoplasty) for comprehensive three-dimensional waist contouring.

4. Post-Operative Recovery, Compression Garments & Clinical Safety

Abdominoplasty spans approximately 3 to 4 hours under general anesthesia, requiring 1 to 2 nights of inpatient monitoring. During the initial 5 to 7 days, patients adopt a slight forward-flexed posture (beach-chair position) when ambulating to minimize wound tension along the suprapubic closure. A customized medical abdominal binder must be worn continuously for 6 weeks to compress seroma potential and facilitate fascial adhesion. Sedentary work resumes at 10 to 14 days, with intense abdominal core exercises cleared after 8 weeks. In Turkish Ministry of Health-certified facilities, pneumatic compression boots, anti-thrombotic chemoprophylaxis, and early ambulation ensure utmost safety.
