What is Androgenetic Alopecia? Understanding Genetic Hair Loss

Author: Op. Dr. İsmail Büyükçayır · Published:

The leading etiology of hair loss in both men and women, androgenetic alopecia arises from progressive follicular miniaturization triggered by dihydrotestosterone (DHT) sensitivity.

Key points

  • DHT Pathway: 5-alpha reductase converts testosterone to DHT, which truncates the anagen growth cycle in genetically receptive follicles, causing miniaturization.
  • Polygenic Inheritance: Hereditary transmission involves multiple loci from both maternal and paternal lineages determining age of onset and severity.
  • Donor Immunity: Occipital scalp follicles lack androgen receptor density, conferring lifelong immunity to DHT and enabling permanent transplant outcomes.

The Miniaturization Cascade and Early Symptoms

Androgenetic alopecia does not present as acute telogen shedding, but as gradual thinning and caliber reduction known as follicular miniaturization. While a healthy terminal hair stays in anagen growth for 2 to 6 years, DHT exposure shortens this window to months. Consequently, robust strands devolve into wispy, unpigmented vellus hairs. Disparity in shaft diameters observed during shedding represents a definitive diagnostic indicator.

Therapeutic Algorithm: Integrating Medical and Surgical Protocols

The definitive standard of care combines medical stabilization with surgical restoration. FDA-cleared 5-alpha reductase inhibitors (Finasteride/Dutasteride) suppress DHT production to defend existing native hair, while Minoxidil enhances follicular perfusion. In permanently bald zones, FUE or DHI surgery replenishes density using DHT-resistant occipital grafts that retain donor dominance permanently.
